Mamma Rh negativo e profilassi anti D I problemi di incompatibilità materno-fetale più frequenti dipendono soprattutto dal fattore Rh, per questo le donne con questo fattore devono sottoporsi mensilmente al test di Coombs indiretto, un esame che individua gli anticorpi anomali presenti sulla superficie dei globuli rossi materni. Questo esame è particolarmente importante per le madri Rh negativo in quanto, se il figlio fosse RH positivo potrebbe sviluppare antigeni contro i globuli rossi del bambino che a sua volta potrebbe sviluppare la malattia emolitica del neonato. Negli ultimi anni in Italia, come già accade in altri paesi, si somministra l’immunoprofilassi anti D di routine tra la 28esima e la 30esima settimana a tutte le donne Rh negativo, pratica suggerita dalle Linee guida per la gravidanza fisiologica del Ministero della Salute. La somministrazione di immunoglobuline anti D è utile in quanto si è osservato che un consistente numero di donne Rh negativo produce anticorpi per feto Rh positivo già nel terzo trimestre di gravidanza quindi si preferisce una somministrazione precoce oltre che quella dopo il parto.
